Client Forms & Practice Policies
To support informed, ethical, and effective care, Aligned Living Psychological Services (ALPS) uses several standard forms and policies for all clients. These materials are designed to explain the nature of services, outline client rights and responsibilities, and ensure transparency regarding fees, privacy, and clinical procedures.
All required forms are provided through our secure client portal before the start of services. Existing clients can follow the link to access their secure portal, while new clients can submit an inquiry form to request a consultation.
Required Client Forms
Clients who choose to move forward with services will receive a set of required intake forms through the secure client portal. These forms must be reviewed and completed before scheduling or beginning services.

Forms are completed electronically through the portal to protect confidentiality and ensure accurate recordkeeping.
Good Faith Estimates
Clients receiving self-pay services are provided with a Good Faith Estimate as required by federal law. This estimate outlines the anticipated cost of services based on the course of treatment discussed in your consultation call or intake appointment.
Clients are asked to acknowledge receipt of the Good Faith Estimate through the secure client portal prior to beginning services. Updated estimates may be provided if the scope or nature of services changes in a way that affects anticipated costs.
Privacy and Confidentiality
ALPS is committed to protecting the privacy and confidentiality of your personal health information. Our Notice of Privacy Practices explains how information may be used and disclosed in accordance with applicable federal and state laws, including HIPAA.
The Notice of Privacy Practices is provided to all clients through the secure client portal and must be acknowledged before services begin.
Practice Policies
Practice policies outline important information regarding scheduling, cancellations, fees, communication, emergencies, telehealth services, and other administrative procedures. These policies are intended to promote clear expectations and support a professional therapeutic relationship.
Clients are asked to review and acknowledge practice policies through the client portal prior to initiating services.
How Forms Are Completed
All intake forms and acknowledgments are completed electronically through our secure client portal. Forms are not completed by email or through the website in order to maintain privacy and comply with confidentiality requirements.
Clients are encouraged to review forms carefully and discuss any questions or concerns with their provider before beginning services.
After an Initial Consultation
Following an initial consultation, clients who decide to proceed with services will be invited to the secure client portal and provided with all required forms. Services are scheduled once the required forms are completed and any applicable deposits or payments are received.
