Good Faith Estimate for Healthcare Services
In compliance with the No Surprises Act (2022)
Under federal law, clients who are uninsured or choose out-of-pocket care have the right to receive a Good Faith Estimate (GFE) explaining the expected cost of mental health services.
You are entitled to:
Receive a written Good Faith Estimate before your scheduled service.
Request an estimate at any time, even if you are not ready to schedule.
Receive a revised estimate if your treatment plan or fees change.
Dispute a bill that is at least $400 more than your Good Faith Estimate.
Receive care without surprise or hidden fees.
What Your Good Faith Estimate Includes
Your personalized GFE will outline:
The type of service you are seeking (e.g., therapy, psychological testing).
The length and frequency of your anticipated appointments.
The provider’s hourly rate or service rate.
The estimated total cost over 12 months, based on clinical recommendations.
Any additional anticipated testing fees, if relevant.
Instructions on how to request a revised estimate.

Limitations of the Good Faith Estimate
The GFE is not a contract, and receiving one does not obligate you to obtain services.
Actual costs are subject to change if:
You attend more or fewer sessions than estimated
Your provider adjusts the treatment plan
Additional testing or specialty services are recommended
You may request an updated estimate at any time.
